This portfolio of tools is designed to help long-term and post-acute care (LTPAC) organizations plan for and select shared care planning and coordination technology.
About Shared Care Planning and Coordination
Shared care planning and coordination involves creating a comprehensive, personalized written statement of a care recipient’s assessed health and social support needs. This care plan identifies the supports that providers and caregivers should deliver, why and when, and who is meant to provide it.
The ability to communicate with and educate older adults in the home is an important feature of care management. Technology that accomplishes and/or facilitates these goals can provide substantial benefits, according to CAST.
The portfolio includes several components:
Whitepaper Exploring Shared Care: A whitepaper entitled Shared Care Planning and Coordination for Long-Term and Post-Acute Care: A Primer and Provider Selection Guide helps organizations understand the range of issues surrounding shared care planning and coordination; outlines technology solutions available in the marketplace, as well as their uses and benefits; and assists providers in planning for these technologies.
Interactive Guide: An interactive guide is geared to busy executives who want to quickly navigate the comprehensive whitepaper. The guide offers a high-level overview of the issues, technologies, and planning process.
Product Selection Tools: A Product Selection Matrix matrix compares 18 different shared care planning and coordination technology products from 17 vendors (also available in Excel format). In addition, an easy-to-use Online Selection Tool helps providers identify appropriate products that fit their care settings and offer their “must-have” features.
Case Studies: Five case studies provide real-life examples of how LTPAC successfully implemented shared care planning and coordination technology.
