The pressure ulcer risk assessment is mandated by CMS to be part of every facility’s skin and wound prevention and treatment program. Research has demonstrated that the incidence of pressure ulcers has decreased by as much as 60% when a pressure ulcer risk assessment tool is properly administered.
This session will focus on pressure ulcer prevention with emphasis on a comprehensive pressure ulcer risk assessment using the original validated documents of Braden Scale for Predicting Pressure Sore Risk
©. It is important to note that often the Braden Score (the number added up) is not the same as the level of pressure ulcer risk, particularly in the population served in the long-term care setting.
Included in this session is content on creating an “individualized” pressure ulcer prevention program based upon an
accurate Braden score.
